Charges were filed against the driver allegedly involved in a car accident that left two people -- including a teenage girl -- dead and a third seriously injured.

Roger Tracy Malloy was charged with "Accident Involving Death and Personal Injury While Not properly Licensed (three counts); Accident Involving Death and Personal Injury (three counts); Driver Required To Be Licensed and Driving While Suspended DUI Related," according to a news release from Montgomery County District Attorney Risa Vetri Ferman and Pottstown Police Chief Mark Flanders.

Malloy was involved in and left the scene of the crash without rendering aid or providing any information to law enforcement, the release states.

"On Monday, May 21, 2012 at 11:29 p.m., Pottstown Borough Police Department responded to a motor vehicle crash with injuries in the area of 520 State Street, Pottstown, Montgomery County, Pennsylvania," the release states. "Upon arrival police observed a white female laying in the roadway on State Street, west of 520 State Street. The female victim had no signs of life at the scene. The female was later identified as Rachel Witt, age 15 of Pottstown, Pennsylvania. A Montgomery County Deputy Medical Investigator pronounced Rachel Witt dead at the location of the crash."

Officers also found two males outside a gold colored Lincoln Continental with heavy crash damage, which was stopped in the alley next to 520 State Street, the release states.

"One of the males was found to be James N. Crawford, age 28 of Pottstown, Pennsylvania," the release states. "The other male subject was found to be Kendal Harper, age 16 of Pottstown, Pennsylvania. Upon the arrival of emergency medical personnel Crawford was transported to Pottstown Hospital for treatment and Harper was transported to Reading Hospital for treatment.  On May 22, 2012 at 12:16 a.m., Doctor Fernandez of Pottstown Hospital pronounced James Crawford dead at Pottstown Hospital. Harper was admitted, in critical condition, to Reading Hospital with head trauma and internal injuries."

An investigation determined two males removed "other occupants" from the vehicle and ran from the scene before police arrived, the release states.

"One of the individuals is alleged to be the driver, Roger Malloy," the release states.

"Investigators also determined from the Pennsylvania Department of Transportation Bureau Of Driver Licensing that Roger Tracey Malloy, DOB 5-8-1985 has a photo ID only. He is assigned a Pennsylvania Driver License, which is under suspension DUI related. According to the driving record the suspension is from an April 29, 2011 DUI offense."

Police ask for the public’s assistance to locate Malloy.

"Anyone with information on the defendant’s whereabouts should contact the Pottstown Police Department at 610-970-6570. The investigation is continuing."
